FFAP will no longer accept Humana.

October 15, 2024

Dear valued patient,

At Family Foot & Ankle Physicians (FFAP), your health is our top priority. That’s why we are reaching out with important information about changes to your health care coverage.

FFAP will no longer accept Humana. This includes the Humana Medicare Advantage plan and all commercial employer-sponsored coverage effective date of December 13, 2024.

We understand this is the season for open enrollment plans for all our patients, which is why we wanted to get this notice to you as soon as possible in efforts for you to make informed decisions for your 2025 health care coverage. Here are a few important dates as it relates to open enrollment periods that may affect you.

Medicare Advantage: October 15-December 7

NC State Health Plan with Humana Medicare Advantage (Retirees): September 30 – October 25

The NC State Health Plan deadline is fast approaching and according to their site, a Base PPO 70/30 plan is administered by Aetna, which is in FFAP’s network.

If you remain with Humana, we are not discharging you from our practice. However, effective December 13, 2024, you will be expected to pay in full at the time of service. We encourage you to contact our office before your appointment to estimate your out-of-pocket costs.

Here is our plan to continue service with you prior to December 13, 2024

  • New Patients: FFAP is no longer taking any new patients with Humana as of October 15, 2024
  • Current Patients with Ongoing Care: If your treatment plan is ongoing with FFAP, we will continue to take Humana coverage through December 12, 2024
  • Current Patients seen for Routine Foot Care: Commonly referred to toenail trimmings; FFAP will continue to take Humana coverage through December 12, 2024
  • Current Patients with a new problem: FFAP will not take any new problems that arise with current patients as of October 15, 2024. For example, if you are being seen for the treatment of a wound, but then you sprain an ankle, the ankle sprain would be considered a new problem. You can call to inquire about out-of-pocket costs for new problems that may arise.
  • Patients requiring surgeries: If it is an elective surgery FFAP will be happy to refer you to another clinic that accepts Humana. If the surgery is critical to your treatment plan, FFAP will provide surgery to you, but must take place prior to December 13, 2024. See your physician to determine if your surgery is elective or critical to your treatment plan.

If maintaining in-network access to Eastern North Carolina’s top podiatry clinic in 2025 is important to you, we encourage you to call your broker to explore your Medicare Advantage plan options during open enrollment. Let them know how important it is for you to maintain in-network access with FFAP. We also encourage you to connect with Centers for Medicare & Medicaid Services (CMS). Learn more about Medicare enrollment and how you can change plans at www.cms.gov or call 1-800-633-4227.

Please know this decision for FFAP did not come lightly. We hope we can retain you as a patient, but if you need our office to transfer any medical records on your behalf, please contact us to complete the necessary form and we will be happy to assist you.

Sincerely,

Your Provider Team at Family Foot & Ankle Physicians
